Please fill out PTSA Check Request Form in it’s entirety.

• Only 5 purchased items can be submitted for request or reimbursement on each form.  If you are requesting reimbursement for additional items, you will need to submit a new form
• All receipts and supporting documents must be uploaded
• Please do not include sales tax in your request(s) Due to the SHS PTSA 501 (c) (3) status, SALES TAX CAN NOT BE PAID or REIMBURSED.
